Out of Network
Often there is a network symbol on the back of your insurance card that enables you to receive health care services out-of-network. We will work with you and your out-of-network benefits or provide a fee schedule for you to see our doctors on an out-of-network basis. If we are out-of-network with your insurance plan, payment in full is expected at the time of service. We will be happy to provide you with the information necessary for submitting your claim to your insurance carrier.If you would like to discuss billing options, please call our Billing Department at , option 6. The billing department is available to take your call Monday - Friday between 8:30am and 5:00pm.
Self-Pay
We also provide medical care to self-paying patients. We will be happy to provide a fee schedule for you to see our doctors on a self-pay basis.
